Please contact us for appointments and prescription refills during regular office hours.

For upcoming appointments:

Please be prompt and if possible, arrive 10-15 minutes prior to your scheduled visit to permit the appropriate information to be collected or reviewed at check-in as well as to allow time for any forms to be completed. Please bring your updated insurance ID card and your copay (if applicable). Copays are due at the time of service.

Please be aware of our Cancellation Policy:

If you are unable to keep a scheduled appointment, please notify us ASAP, as we require 24 hours notice for cancelled appointments. There will be a $25 fee for those who do not keep scheduled appointments.

Questions:

Your questions are important to us. Our medical triage staff is trained to deal with most of medical concerns. If you have questions that were not covered during a routine visit, we welcome your call during our regular office hours.

After Hours:

For serious medical concerns, you can reach our after hours call center by calling our office, (352) 560-7337, and following the instructions on the message. If you feel your child is experiencing a medical emergency, please call 911.

We require all our patients to maintain current vaccinations. Please read our vaccination policy to familiarize yourself with our approach to vaccines.
